Executive Summary
After decades of scientific stagnation, fragmented services, and moral neglect, the U.S. mental health system is overdue for systemic reform. The Challenge laid out the scope of failure: outdated diagnostic models, ignored science, broken policy, and a widening care gap affecting more than 70 million Americans.
This document presents The Plan: a three-stage, time-limited reform strategy led by ARETÉ Science, designed not to tweak—but to replace—our national approach to mental illness with one grounded in biology, data, and dignity. The model draws inspiration from the AIDS reform movement: first build pressure, then force change, then leave behind a new system.
ARETÉ’s role is catalytic. Our mission is to make mental health a national priority—and build the public, political, and structural support necessary for lasting transformation.
- The EMERGENCE Initiative: Reform in Three Stages
Stage One: Awareness, Saturation, and Stigma Disruption
Mental health is underrepresented in journalism, misunderstood in public discourse, and too often reduced to either celebrity tragedy or acts of violence. ARETÉ will change this by launching an integrated media and narrative strategy to elevate real, science-driven conversations around brain health.Key tactics include:
• Creating editorial desks focused on specific stakeholder groups (e.g. families, policymakers, business leaders)
• Producing original content through a Substack column and national op-eds
• Monitoring breaking news to inject science-backed commentary in real time
• Building a clearinghouse of accessible, credible public education resources
• Collaborating with key voices in neuroscience, journalism, and patient advocacyStage Two: Policy, Parity, and Structural Leverage
Once awareness hits critical mass, ARETÉ will mobilize its network to push for legislative and policy reform at both state and federal levels.This stage focuses on:
• Enforcing and strengthening mental health parity laws
• Reforming how Medicaid, Medicare, and private insurers cover psychiatric care
• Advocating for a national strategy that integrates behavioral health into primary care
• Supporting workforce development programs and licensing reform
• Elevating mental health care to the level of cancer, cardiovascular disease, and diabetesStage Three: Blueprint for a New System
The final phase delivers a scalable action plan to replace our outdated infrastructure with a modern, biologically grounded mental health care system.This blueprint will address:
• Clinical infrastructure (beds, outpatient centers, crisis response)
• Integrated care models that connect psychiatry, neurology, and general medicine
• Financing mechanisms that align reimbursement with outcomes
• A national public health strategy that includes early intervention, prevention, and population brain health metrics - A Time-Limited, Mission-Driven Model
Unlike traditional nonprofits, ARETÉ is not designed to operate indefinitely. Its structure is mission-specific and time-bound, with each stage contingent on success in the prior phase. Funding is raised one phase at a time. If outcomes aren’t met, the campaign can be paused or sunset.This approach is designed to maximize:

ARETÉ Science was founded by Dr. Jeffrey Lieberman, one of the world’s leading voices in psychiatric research, care reform, and public advocacy. He has advised on national legislation including the Mental Health Parity Act, the 21st Century Cures Act, and the Affordable Care Act’s behavioral health provisions.This strategy draws on four decades of clinical care, neuroscience leadership, and federal advisory experience—and a deep belief that science and dignity must finally guide our national response to mental illness.
